Back to school is a very busy time with lots of paperwork to keep track of and copies to make! These flip books condense all of those forms into one place, for both parents and students. Together, these 2 flip books will give you a more comprehensive view of your new students.
This resource includes 2 Flip Books + 2 EDITABLE versions!
*This resource will download as a ZIP folder containing two files: one PDF and one PowerPoint. The PowerPoint is editable and is also compatible with Google Slides and Keynote.
Parent Flip Book [Student Information] includes:
-3 words to describe their child
-2 goals they have for their child this school year
-Questionnaire to help you get to know child's academic habits
-Important Info Form (All the specifics such as parent contact info, student's allergies, dismissal method, birthday, etc.)
-Note To Teacher (A place for parents to inform you of any additional information they think you should know about their child)
Parents can complete the Student Information flip book at Meet the Teacher night, Open House, or send it home with students on the first day of school.
Student Flip Book [All About Me] includes:
-3 words to describe themselves
-2 goals they have for themselves this school year
-Questionnaire to help you get to know their academic likes/dislikes
-"My Favorites" page asks them to fill in the blank for their favorite things in each of 14 categories
-Reading Interest Inventory allows you to get to know them as a reader!
-Self Portrait (A place for them to draw themselves as well as things that are special to them)

Students can complete the All About Me flip book at Meet the Teacher night or on the first day of school!

Both flip books include a NOTES section on the back. A blank space for you to keep anecdotal notes from meetings, conferences, test scores, etc. Use sticky notes or write right on the page!
