Daily Work Feedback Form - job skill evaluation

I developed this form for my high school students participating in a school and/or community based work experience. This form is completed daily to gather information about their work abilities such as following directions, accepting direction/feedback, completing tasks accurately etc.

This form would be a great resource for the classroom teacher not able to be on the job site with their students. Job coaches could use this as a tool to determine skills that need to be addressed within a specific work site.

While this form was designed for high school students, it could easily be used with middle school students that are beginning to develop employment skills through classroom or school based work experiences.

*I have several other rubrics and data collection sheets that can be used to evaluate and track students during work experience opportunities.
