Description

With all of the mobility in our society, students run the risk of falling through the cracks when they move and/or change schools. I created this very handy note to put into the cum folder for students who have special needs such as being part of the General Education Intervention process (SIT Process) with a BEHAVIOR PLAN or even students who are in Special Education.

By putting one of these into the cum folder, the receiving school will know that there have been interventions put into place to support the students, what works, what does not work, etc. By offering contact information, it is easy for the teacher or other staff member to make contact and locate these documents so that the student does not miss out on receiving the needed support for success.

Our school is in an inner-city setting, and many of our students have little stability in their lives. Parents sometimes don't know if their child has an IEP, behavior plan, or is being supported by the SIT process or General Education Intervention process. This is a safeguard that is designed to plug the holes in the cracks.

This is fully customizable so that you can make it fit your needs.
