Choir Student Interest Survey and Vocal Range Information

This is designed for use on the first day of the school year to collect information on choir students. It's a great way to get to know your students on an individual level and can aid you throughout the year in making meaningful connections with the students. With this survey, you can also collect parent contact information and add vocal range and part assignments. I send this sheet home with my students on the first day then begin listening to each student's range after I collect them on day 2.
