This includes the failing spreadsheet, the contact form, and the letter home.

Have failing students and sending a progress report isnt enough? Here is a failure sheet you can email or send home. This failure sheet includes both English and Spanish. It lists a students grade, percentage in class, missing assignments, attendance, number of classes failing, and the amount of help outside of class they have taken advantage of - for us this is called 8th hour but can be changed for your own schools vocabulary.

This also includes a sheets that can be used to plug in information to keep track of it. The sheet directly correlates to the letter so it is easy to fill in the letter. There is a way to have word send the letter using the sheet if exported into excel and if emails are added - that is a little too advance for me at this point

The final item is a form that can be used once the email has been sent or the student has taken home the paper. It allows you to keep track of parent contact for your own records.
