Description

This checklist was designed to help you gather information about a student’s language and play-skills during an informal play-based observation.

I typically use this checklist to help me keep organized and gather more information about a student during early childhood evaluations or observations. The checklist is not meant to replace an assessment/evaluation for your students but simply keep your observations organized in a checklist manner. This checklist has not been normed on other children therefore you will not obtain a score. Please use your professional judgement when using and gathering information while utilizing this checklist.

An English and Spanish version of the checklist has been included. Please feel free to email me if you have any questions.



This checklist includes the following in both English & Spanish:

◙ Section for General Observations

◙ Section for observations of Nonverbal Communication (i.e., gestures observed, facial expressions, etc.)

◙ Section for play-based skills observed (i.e., parallel play, onlooker play, etc.)

◙ Section for observation of Attention Skills (i.e., joint attention, eye contact)

◙ Expressive Vocabulary Checklist (i.e., clothing, body parts, farm animals, food, colors & transportation items)

◙ Receptive Vocabulary Checklist (i.e., clothing, body parts, farm animals, food, colors & transportation items)

◙ Receptive Language Checklist (i.e., responds to name, actions, commands, spatial concepts)

◙ Answering Questions Checklist

◙ Section for observations of Articulation/Phonological Processes

◙ Communication Intents observed (i.e., requesting, commenting, seeking assistance, etc.)

◙ Communication Method Preferred Checklist

◙ Section to gather a Language Sample with MLU formula
