I use this in hopes that it will prevent students from reoccurring behavior issues. I make them fill this out as a consequence during activity, instead of participating in activities with the rest of the class.
This is an easy way to have students reflect on their behavior and choices. Lends well to being able to have a meaningful conversation with them afterwards.

These behavior reflection sheets are intended to be used with your own personal classroom management system. I use these reflections as a consequence for breaking classroom rules. 3 warnings result in a teacher choice consequence, such as classroom cleaning and then a reflection either during recess/ break time or while the rest of the class take part in a fun activity. They are intended to encourage better choices and a clearer understanding of your students’ behavior.
I’ve included an optional note to send home to include parents in the process.
