Behavior Monitoring Sheet

Many schools, now a days, are not okay with a public display of a students behavior record and so I created this more private way of monitoring a students behavior through out the day. When a child misbehaves pull them aside show them the file folder and their name.

Two ways of doing this.

Option One: Type in the students name, print out, glue into the inside of a file folder, laminate, and use a wet erase marker to check off where the child is throughout the day.

Option Two: Type in the students name, print out, glue into the inside of a file folder, laminate, and place mini velcro circles onto every little rectangle sort of like the attached image. If by chance the mini velcro circles don't quite fit you may need to resize the cells.
