Autism Guiding Questions Form

This form is a set of questions, aligned to the DSM-V diagnostic criteria for Autism. It is a simple questionnaire that teachers, administrators, counselors, evaluators, and parents can use to easily run through the requirements of an autism diagnosis, in an organized and visual manner.

I use this during RtI and referral meetings to collect brief information from both staff and parents, to help illustrate why an autism evaluation is or is not warranted.
