This is a brilliant form for enticing students to share about themselves. My students were required to share and encourage each other during the ice-breaker activity. In no time, they were laughing and engaged. I highly recommend this product for use with high school students.

This is not your average first day of school survey! Tested in my own high school classroom, this is an excellent first day of school information sheet that will help you get a glimpse of who your students really are. I suggest you discuss the questions that most interest your students as a class. I have had some pretty engaging and interesting classroom discussions using this survey, especially for the first day! You could also introduce the activity by sharing your own answers to the survey. Overall, I have had a lot of success using this survey and hope you will as well.
