Description

As teachers, we want to help all students learn. Many times, teachers give accommodations to students other than special education students. These students are usually in the Response to Intervention (RTI) program. It is important to come prepared to student meetings with documentation of the accommodations that you have given so that you can give a recommendation for an increase or decrease in tier level. Sometimes, you need to come ready to a meeting requesting a referral to your school's Special Education program. You will be able to show direct correlation between an accommodation given and the grade earned by the student. These forms have been made to help you keep track of accommodations that you give students who do not have an IEP.

What do the forms offer:
*Quick check-box format to indicate accommodations given
*A space to indicate what type of assignment (test, classwork, homework, project) to which the accommodation was given
*An area for reflection on the success of the accommodation
*An "other" accommodation selection to add unique accommodations
*Accommodations included in check list: extra copies, extended time, corrections, retake, read aloud, shortened assignment, notes/study guide, re-teach (small group), re-teach (one on one), other

There are two types of forms in this document. The first form is ideal for the middle school or high school teacher who has many students and cannot have one form per student. The second form is ideal for the elementary teacher who can devote one page per student. There are two copies of each form to allow you to print front to back easily.

How I use them: I have my accommodation logs in a binder with my grade sheets and parent communication forms separated by tabs.
