Absent Work Form for Students

How do make sure absent students receive all of their work and have adequate instructions to complete their absent work? This form is life saver for me. I keep it saved on my desktop and I fill in the information at the end of the day for the absent students. All of the instructions and directions are there for the student and parents to see. It is also a great way to just duplicate the sheet and information if you have multiple students absent that day! There is no need to write information twice.
