Description

*UPDATE*: I've decided to put this in Word format so you can edit the categories to fit the issues you are dealing with.

This behavior checklist would best suit a child who has already been diagnosed with ADHD or has the same symptoms as ADHD. It isn't a screener to identify possible ADHD.

I have personally used it for my 8 year-old son for a few months now, and it has helped him to be more conscientious of his actions throughout the day.

I designed this daily behavior checklist to help enforce independence and responsibility to the child. The student has the duty of filling the sheet out, then the teacher simply okays it at the end of the day, and or provides any notes necessary.

The idea is that the paper is reviewed by the teacher at the end of the day and signed, then brought home so the parents can know how their child behaved that day. The child has to discuss it and why they marked their behavior as such. It's very simple and not time consuming- it can all be done at the end of the day. It also makes it easier so that the teacher does not have to take so much time out of their day to focus on one student, but is able to participate in helping the child.

So you can get an idea, a few things that are included on the sheet (but in more detail) are:
Arriving at school
Interrupting & distracting
Following rules
Voice level
Organization
Focus
End of the day

*Also, in the thumbnail preview the squares don't look the same size, but they are. That would bug me if they weren't :)

I'm charging $2.00 for my files because I'd like to be able to make and sell more things to help parents and teachers that have children with ADHD, and this way I will not have to do it just in spare time. Thank you!
