3rd Grade End of Year Student Survey

This is an end of the year reflection for students to fill out on their experience in your classroom. I've used this form the last three years in my classroom and have had invaluable feedback from students! It helps drive my next year with changes I need to make, and things to keep the same. I am very specific with my students in that this activity is feedback solely for me as a teacher, and is intended for them to give me their true thoughts and reflections.

As an option to save paper, you can just print the front page and explain to students to use the back for more ideas. You can also make a copy to add to your Google Drive account, and have students fill in this assignment online using the add text tool.
