1. Create a goal and jot reasons for wanting a healthier you
2. Record your start date, weight, and BMI
3. Record your end date, weight, and BMI
4. Record beginning and ending measurements for hips, waist, and chest
5. Check off the days you are able to stay on track
6. Mark the days you exercise
7. Record self-care for each month. Treat yourself for your hard work!
8.Write down your successes and your challenges and what you notice as you become healthier!
