It was created in Microsoft Publisher so you can change/delete/add text and graphics as long as you can bring Publisher up on your computer. It is a frog theme but can be changed to any theme. The frog theme may not translate to your computer if you do not have the font but I have used it with the plain letters and it is just as cute.

This is an accountability piece that I use in my 4th grade classroom. Students fill one out when they do not do their homework. It makes them write WHY they didn't do it as well as identifies which Character Pillar they chose NOT to follow. Included is a place for parental signature and date. When the student returns the completed assignment, I clip the coupon to it and send home with the final grade. I keep the top portion for my records to help with work/ study grades.
