Family Reading Survey
This is a simple, but great survey that helps a teacher to understand the families' literacy backgrounds as well as the materials and resources that are readily available for the children to read at home. This will help teachers gain the knowledge to better support their students' literacy experiences and potentially help supplement what they are not receiving in the home.

Book Club Permission Slips
These book club or library club permission slips are a great way to keep track of club membership. It is also a good way to protect yourself from any liability issues, so I recommend keeping the signed forms in a file drawer. This looks best in color, but black and white is just as effective.
