Description

Get organized!!! If your students need help being organized then this document is for them. It's a weekly form that they fill out and you quickly score. Great for student accountability and teacher monitoring ease. I use it for a study skills and study hall class to help my students keep up on their current and missing work.

If students score a 40/50 then on Fridays I let them bring in food to eat or get something from my goody area as their incentive.
