This Google Form contains over 30 questions perfect for secondary students, and are divided up into three sections. The three sections on this form include general information and get-to-know-you questions, learning style, and student skills. In the general information section, there are questions about what students want to be called in your class, what they would like to do in the future, fun facts about them, their goals for the year, how their teachers from last year would describe them, and more! The learning style section asks questions about how they learn best, how they collaborate best, and what assignments or assessments they prefer. The final section assesses how comfortable they are with various class activities or skills.
The questions are designed to be used with any content area at the middle school level. Better yet, since this is a Google Form it is completely editable, so you can add or remove any questions you want. Going paperless with this student interest survey has never been so easy!
This resource is listed with the Google Apps option on TPT, which means this file will download directly into your Google Drive account.
