Description

This form can be used to plan and prepare for parent conferences.
This form would be given to the student prior to a parent teacher conference, IEP or RTI meeting. The students can provide written input behavior and academics strengths and weaknesses.

As the Learning Resource Teacher at my school, I hold annual review parent/teacher meetings for all students on academic support plans or students that were on intervention. I had students complete this form before the meeting. I was able to review their comments and prepare for the meeting and share it with all teachers, parents and professionals at the meeting.

Parents appreciate such preparation and seeing that their child was able to reflect and contribute to planning for success at school.

This form is kept in the child’s file, which makes a nice addition to portfolios or reflection form year to year.
