Description
Do you need a simple, easy way to obtain academic and behavioral information for students in your school?
This evaluation tool will provide you with the information you need to write and support clear data-based Individual Education Programs (IEPs), present levels of performances (PLOPs), data-supported special needs Functional Behavioral Assessments (FBAs), and Behavioral Intervention Programs (BIPs). It is also a helpful tool to assist in gathering observational data for Child Study and RTI plans.
Overview of the Teacher Rating Report
These teacher rating reports are formatted using the Likert scale and cued responses. This rating scale has been tested over a five-year period in Grades 6-8. When compared with an open-ended questionnaire, teachers prefer this format because it provides qualitative, quantitative, and subject-specific data in an easy-to-use and clear way. My return rate from teachers drastically increased when using this format. Teachers are more positive about completing them and are able to do so faster. These reports can also be customized for the teacher by highlighting the sections where their input is needed.
The results from this report allow you to compare behaviors of concern across classrooms as well as calculate an average rating for each targeted behavior. Two open-ended questions are included so that teachers can add a personal opinion about the student’s strengths and areas of concern.
A variety of subjects are covered in these reports, including academic subjects such as Class Rules and Behavior, Reading, Phonics, Math, Written Language, Science, and Social Studies. Executive Function Skills, Testing, and Accommodations are also covered to provide a comprehensive report for the student.
