I created these teacher interviews to help SLPs in our district to help determine educational impact for students we treat and those we are evaluating. We have found that using these interviews, as opposed to interviews with simple check boxes, has helped us to obtain more specific, thoughtful information from teachers.
I thought you may be able to use them in your practice, too! Use your professional judgement to determine IF and HOW speech deficits are impacting your students at school based on their teacher(s)' answers. Also, it's ideal for helping to develop IEPs, to plan targets in therapy, and to make evaluation and re-evaluation decisions.
