This is a mental health check in and physical self-help health questionnaire or survey to get your students (or you) to reflect on your diet, sleep, screen use, exercise and more! This can be a piece of your health science curriculum.

There are five pages of questions with space for written answers. There is a sixth page that you, or your students, can make copies of and use as a goal-journal, to help achieve daily health goals!

The questions ask about overall health, diet, sleep, exercise, screen use, social life and health issues one may have. There is space for the respondent to ask health questions, too. There are questions that ask the respondent to rate their health on a scale of one to ten, with ten being the highest, and there are questions that ask for written answers.

This is great for classroom use, in your life skills, physical education, health class or your English Language Arts class, or for a homeschool. This is also great for self-study. This is great for middle school, high school or adult students. This works for ESL students and native English speakers.
