Self Evaluation for Parent Student Teacher Conferences

Students need to become responsible for what they do (and don't do) in the classroom. This self evaluation helps the student look at their own work and attitude in the classroom. They must identify what they are struggling with and think of a way to improve that weakness. I use this for parent teacher conferences. I don't mind conferences anymore because my students do most of the talking. (They are usually harder on themselves than I would ever be).
