Positive and Negative Behavior Management Form

Behavior Documentation Form includes:

• a form for the teacher to document specific student in a behavior management folder

- date, step taken/behavior description. student signature, notes/parental
contact

• a form for parental contact with check list of common behaviors

-negative behaviors (cell phone, inappropriate language, etc.)

-positive behaviors (encouraging others, takes leadership role, etc.)
