Planning for My Future {Transition Planning}

Planning for life after High School can be incredibly challenging for students with special needs, yet preparing our students for success and including students in the planning process is so important! this resource give you and your students tools to identify and develop transition goals as well as identify possible careers to explore!

This Resource Includes:
1. Parent Input Form - this 3 page form will help you to gather information about current skill levels as well as student and parent goals and preferences from the perspective of the parent(s)/guardian(s).
2. Student Forms (2 versions - 1 with visuals and 1 without) - These focus on the following topics/questions:
-Where do you want to live & who do you want to live with?
-Taking care of myself & my home
-Where do you want to work & what types of jobs do you want to do?
-Who is important to you?
-How will you get around?
-What will you do for fun?
