Description
This Communication Log is designed to document all communication that you have with a student's parent or guardian. The form is designed so that you have one form for each of your students. You are able to document the following information in this Communication Log:
*Student Info. (name, parent name, phone #, willing to accept texts, and email)
*Date / Time of contact
*Method of contact - phone, email, in person, or other
*Reason for contact
*notes
