Description
This NO PREP product is flexible and adaptable to help organize your therapy session data and allow for easy progress monitoring.
Service providers should be doing their due diligence in maintaining records of speech therapy sessions with their students. This not only benefits the SLP, SLPA, OT, PT, etc. allowing them to look back, remember, assess skills, and prepare for future sessions, but also helps guide curriculum and/or goals by establishing a baseline and further progress monitoring options for their students.
This easy, editable Google form will make it quick to jot down specific details and ideas after each session!
It also provides tangible data and organization for student's, allowing educators to review previously completed forms!
How-to: Simply click the three dots in the right hand corner of the google doc. Click "Make A Copy". Title the copy with the STUDENT'S NAME and specify which folder to add their notes to. Click "Make A Copy" again. Now, each time you want to complete notes, you will need to access the link to this newly created copy for the google form, and fill out the new information (date, session time, etc.). In order to do this, you will click "Send", then "Copy" the LINK for this Google Form.
** I recommend you keep this link in a separate document to access easily. **
Before long, you will have a running log of the year's therapy notes specific to each student!
Have another student addition to the case load? No problem, simply create another copy with the new student's name and information, copy that NEW google form link, and access that specific Google form for each additional notes!
WHAT’S INLUDED:
Access to Google Form via Google Drive - Ready-to-use (after making a copy). Once a copy is created in your own Google Drive, you can continue to make copies for however many students you have on your caseload!
Not a fan of Google forms? No problem! Simply click the three dots on the right hand of the Google form copy, then click "Print", and either save as a PDF or print worksheets of these copies for each of your student's folders!
