My students fill these out on the first school day of the month: Reflecting on the previous month and setting a goal for the new month.

Sample self-evaluation questions:

i) J’apporte mon agenda à la maison tous les soirs et je le ramène à l’école tous les matins.

ii) Je remets mes devoirs à temps.

iii) Je démontre la maturité en faisant de bons choix.

iv) Je commence mon travail rapidement, sans rappels.

Self-evaluation questions are divided into the 6 Learning Skills & Work Habits categories from the Ontario Report Card:

- Responsibility

- Organization

- Independent Work

- Collaboration

- Initiative

- Self-Regulation

Students rate themselves based on the E G S N scale: Excellent, Good, Satisfactory, Needs Improvement.

There is a space at the bottom for 3 signatures: student/ teacher/ parent. This way, we all know what the student's goal is for the month and can work together to help him/her achieve that goal.

Used in my Grade 5 French Immersion class.

Suitable for Grades 4 to 8.

Adaptable for Grades 4-12 (for older students, I'd change the wording from "Je garde mon pupitre propre" to "Je garde mon casier propre", etc).

This product is in French. Perfect for a French Immersion or Francophone class.
