Mental Health Student Survey - *Editable* - School Counselor

This Mental Health Student Survey can be used in the beginning of the year to capture what your students feel about their school climate. It includes 8 questions that can be answered with "agree", "disagree", or "unsure". There are also open ended questions, such as "I think students need help with..." and "comments/concerns" for students to write in their own ideas. This is a key component of ASCA's data-driven comprehensive counseling program.
