This is a Microsoft Forms Parent Survey that asks questions regarding their student’s current grade in your course. It can be used with a desktop computer, laptop, tablet or smartphone.

This Parent Survey allows your student and their parent/guardian to analyze their current grade and reach out to you with any concerns they may have. This helps keep everyone accountable and will hopefully improve student achievement throughout the year.

You will share a link with your students (via email, class website, or QR code) and receive immediate results upon each submission.
