Lexia Goal Sheet & Data Tracker

In my use of LEXIA, I have tried a number of ways to provide students a method of self-tracking their progress.

This goal sheet allows them to make a goal for each week in the quarter, as well as mark how they are actually doing.

You can have your students mark what level they are at or mark their units completed.

The top row includes all of the levels in LEXIA. It is to be used to date when the student achieved each level.

Please let me know if there is an edit or change I can make that would better suit the needs of the students.
