How to use:
The Inclusion Support Documentation Logs were created for Case Managers to complete about the students on their caseloads. This inclusion is a great way to quickly collect inclusion data and communicate with staff about inclusion services. 

1. Open the Inclusion Support Documentation Log that you want to use in Google Docs. This is your master doc so you will want to create a copy for each data sheet you want to create.
2. Make a copy by selecting “File” in the google doc menu and selecting “create a copy” under the File Menu. 
3. Rename the new copy for the student and save it to a file in your google drive.
4. Fill in the information at the beginning of the table (student, date, etc..) by moving the cursor to each part and typing in the field. Do the same for additional comments and you can add or delete any columns you don’t want to use. 
5. Complete for each student
6. Recording data: Move the cursor to the column of where you want to record data.

This document is great for:

• Creating data files and binders for individual students or small group sessions in the inclusive classroom. 
• Paraeducators can easily use these forms to track data.
• When services change for a student and you need to collect new data.
