Full Individual Evaluation (FIE) Checklist

I create an evaluation caseload binder each school year that includes initials, re-evaluations, FBAs, and psychological consultations. For re-evals, I put checklists in the binder at the beginning of the school year. For initials/consults, I put a checklist into the binder as soon as I receive the referral. I use one of these checklists for each student/evaluation. I then organize them by due date. This checklist contains information I need to access daily so I am able to quickly determine which parts of evaluations I am working on. I refer to it when sending out protocols, receiving protocols, waiting for protocols to be returned, and planning for evaluations.

This checklist is a great resource to have throughout each day at work and at home. it is an easy and fast way to access all information I have for an evaluation, what information I still need, as well as upcoming due dates (without having to carry around all of my folders!).

Each morning I look through my evaluation checklists to see what pieces of an evaluation need to be completed. Once I have completed as much as I can for a given evaluation, I turn the page to the next checklist (which would be the next evaluation due when organized by due dates) and so on.

School Psychologists, LSSPs, and other school evaluators would benefit from this form.
