If you have ever been through the loooooong IEP process, you know how time consuming and paperwork-heavy it can be. In order for our students to be found eligible, we must have collected enough data. As a classroom teacher, it can be almost impossible to have time to take down good data while still teaching all day. With this form, all that you need to do to track is put a check mark in a box!! Simply add in the behaviors that need tracking and the dates (option of a full 2 week tracker or just week-by-week) and then put a check in the box each time that behavior is observed. There is also a spot for notes for each tracked day (if needed) that you could further color-code. I have been using this sheet for years now to take to IEP meetings and show my data. Each time, meeting members have always appreciated the format and easiness of readability. This product also includes a filled-out example so that you can see how it works.
