I teach middle school, and I like to have all my students fill out these sheets and present them to the class as an icebreaker on the first days of school. I attach pictures that I take of each student on the first day of school, and then I put them in a binder by class period. Students like how I remember all this information about them each year. But, in reality, I refer back to these sheets to help with lessons and such to make sure I appeal to my students. I even refer to these profiles to help me get students special prizes throughout the year.
