Diabetic Diabetes Individual Health Care Plan

Developed by a certified school nurse, this two page document outlines the responsibilities of the parent, teacher and school nurse. It provides a quick snapshot of the student's needs, what to look for and how to respond. I give a copy to the teacher to keep in their sub folder, to the bus drivers, and all other staff that come into contact with the student.
