Description
This 1-page Detention Notice or Detention Slip template is designed for upper middle school and high school teachers. The notice is useful if you need to communicate the day, time and reason for a detention with parents, verify that they have received the information, and don't want to spend a lot of time writing!
This detention notice is designed to require a minimal amount of writing by the teacher, taking less than a minute to fill in, while still offering enough detail regarding the detention to both the parent(s) and the student. The detention notice is small, (less than half a page) which allows for 2 detention notices per page. The detention notice comes in a static PDF and editable WORD file, making it fully customizable.
The detention notice provides the following information:
- Week (circled; for schools with a 2-week schedule, or otherwise)
- Day of the week (circled)
- Time of detention (circled)
- Student name (written)
- Day detention was assigned (written)
- Brief reason for detention (circled)
- Parent signature for verification (written)
- Teacher signature for verification (written)
Philosophy/How I Use This Product:
Before creating this product, if I assigned a detention to a student, I would write a detailed note to the parent(s) to communicate the time, location, and reason for the detention. Although I could have easily sent an email, I prefer to have the student present a written notice to their parent(s) to be signed because this better ensures that face to face student-parent communication has occurred.
This handwritten note would take some time to create, so in order to be more efficient, I designed this detention notice to require as little writing as possible. Due to the design, a custom detention notice can be created by the teacher in less than a minute! I print these detention notices on brightly colored paper, cut them out (2 per page), and keep a stack of them in my desk where I can get to them quickly.

Left Side - Detention Information (Circled by the Teacher):
- School Week
- Day of Week
- Time of Day (also offers an Other: _____ choice)
- Detention attended or not attended (for follow-up)
Right Side - Student Information (Filled in by the Teacher):
- Student name
- Day detention assigned
