DIBELS DRA DSA Literacy Assessment Growth and Loss Data Reporting Form

This data reporting form was a creation of mine in response to the demand for growth and loss reporting with regard to student data. I needed a form that was simple to use, well organized, and that showed growths and losses for the data measures being used at my school. This particular form covers the reporting of Developmental Reading Assessment (DRA), DIBELS (specifically Oral Reading Fluency and Word Use Fluency), and the Developmental Spelling Assessment (DSA). However, it can be edited and tailored to any assessments that a teacher gives - test names can be changed, columns can be added or taken away, etc. Included with this resource is a sample page from my 2nd grade class for which the form was created; following the sample page is a blank template to use and edit. This document simplified the data reporting process for me, was easy for my superiors to read, and accomplished the goal of reporting gains and losses. This listing is for a template that you will edit yourself. It has spaces for up to 30 students.
