Do you constantly get requests for speech-language screenings? I sure do! I’m stopped while picking up students, during my prep, and worst of all during my lunch of my 2 seconds break in my schedule to run to the restroom! Worse part of it all is by the time I’m back to my room I’ve already forgotten all about the request, student, or teacher that approached me! This is why I’ve created this screening request form and have implemented it throughout my school buildings. I hang it outside of my office and in the main office right by our sign in book! This way coworkers have the ability to request screenings at any time during the day. In addition to not having to hunt me down, everyone has access to it and it's kept in not one but two central locations making it easier for everyone!

A good documentation log is a MUST for all therapists!

This log has a place for you to keep a continuous log of student issues and teacher contacts/requests. Space is provided to indicate the date of the occurrence and write a short summary of the incident. I’ve learned to keep a digital copy for myself where I can write in any additional information which could be useful for AIS/RTI meetings and other building support and related service providers. 

This screening packet allows the teachers to gain insight on

1. speech and language definitions, domains, and disorders to help educate teachers and help them in being able to identify what area(s) of communication the student demonstrates difficulty

2. checklist for teachers to document areas of concern related to student

3. screening request form/sign up sheet

. It was designed to engage classroom teachers in the screening process. Additionally, as part of the Response to Intervention process, this form is beneficial for sharing knowledge so students can have their needs met in the least restrictive environment.

The purpose of the checklist if for teachers to fill it out when they are requesting the screener. This gives us background information on current deficits and concerns regarding the students performance. If student requires a full evaluation or goes to RTI/AIS it provides those committees with information on students areas of weakness.

The goal is for teachers to complete the checklist form prior to filling out the request form. After the speech therapist has completed the screening they can staple the checklist form to the screening and have it for records.
