Clinic Pass Labels - Nautical Theme

If you're anything like me, you probably get so tired of writing notes to the nurse constantly! I created these clinic passes to be printed on labels so that you can simply fill in the spaces/check the boxes, and stick them in the student's agenda! No more writing lengthy notes, taping/stapling, etc. to take up your valuable teaching time :) If you place one of these in a child's agenda or homework folder (or whatever you use to communicate), it's a very easy way to communicate with parents and let them know that their child had to visit the nurse that day and why.

This PDF file is formatted to print on Avery 5163 shipping labels (10 per page). However you can also just print it on paper and cut the passes apart.

Each pass has a different watercolor nautical clipart (or two) behind the wording. The pictures are set to be 50% transparent so that the words can still be easily read.
