Classroom Library Sign Out Sheets, Reminders, Binder Cover (Check Out)

This resource contains TWO styles of classroom library sign out sheets. One contains a column for the teacher to initial, and the other does not. Students write in their name, the title of the book, the date they checked it out, and the day they returned it.

I use a binder to keep all my sheets together in one place. Therefore, I have included THREE styles of cover sheets for a binder.

Also included in this resource are THREE styles of reminders. Two are for parents. One of the parent forms contains a signature line in case you want to make sure students have indeed shown the notice to their parents. The other style is just a reminder note for the student. They can staple it into their agenda as a reminder.
