Chorus Job Application

With all the extra emphasis placed on adding reading and writing into all curriculums, including the arts, this is one of the ways I have found to add it into chorus. Starting with their signup papers. I have also found that students that want to be in chorus will fill this out, while others that just want to get out of class won't take the time. This document lays out all student expectations, what students can expect from teacher, what to wear for concert, teacher contact info, concert dates, etc.. It also gives me a chance to get to know the students when they answer question 10 and tell me why I should pick them to be in chorus. In Question 12 they sign it and so I also use it as a contract to them agreeing to the expectations. It's also nice to get parent feedback in question 13. My first year of teaching I had a very generic signup form and had major discipline issues all year. I have been using this format for the last 10 years and have had very few students that joined for the wrong reasons. Enjoy!
