Beginning of Year Choir Survey

This is a single page worksheet that asks students for basic vocal information including their name, age, grade, preferred pronouns, years of choral experience, and voice type (bass, baritone, tenor, alto, soprano, or unsure). Then it asks some get-to-know-you questions, and ends by having students write a letter on the back of the worksheet introducing themselves to you or writing about their musical goals for the year.
