This is a resource to help students with Attention Deficit Hyperactivity Disorder become more self-aware. Having the student rate themselves gets them thinking about their behaviors. Having a teacher rate the student helps the student recognize what they need to work on. It also helps them recognize what they are doing well at. This can also be used as a progress monitoring tool.
